REQUIRED HEALTH INFORMATION
PHYSICAL EXAMINATIONS
Each student needs to have documented by their primary care provider that they have had a physical examination within 12 months prior to entering school or within 30 days after school entry.. Documentation of a current physical examination is REQUIRED for all students entering into preschool, kindergarten, and fourth grade.
LEAD POISONING SCREENING
Each student entering Kindergarten must present documentation of a lead poison screening at least one time between the ages of 2 and 5 years.
PRESCHOOL VISION SCREENING
Preschool vision screening must be completed by the primary care provider upon entry to Kindergarten (within the previous 12 months), or within 30 days of the start of the school year: certification that Kindergarteners have passed acuity and stereopsis screening is required.
IMMUNIZATIONS
Immunizations are a vital communicable disease control mechanism. State regulations require that documentation of a student's immunization record is up to date for their grade level requirements. Grade level requirements are listed on the chart below entitled "Massachusetts School Immunization Requirements for School Year 2014-2015.
Exemption from immunization requirements, per Massachusetts law, is acceptable only with documentation by a physician that in his/her opinion the child's health would be endangered by administering the immunization or that the parent/guardian has a religious belief against vaccination. Such certification shall be submitted at the beginning of each school year to the school nurse.
